I agree that before I become a member of Mission94, I must complete and submit this membership application and pay the fee associated with my chosen membership level. In order to become a member of Mission94 after submitting this membership application, I must be at least 18 years old, accept Mission94’s membership agreement* and execute an acceptance of risk, waiver, and release. To satisfy these conditions, I must visit Mission94 in person to verify my age and execute Mission94’s membership agreement, and execute an acceptance of risk, waiver, and release.
I understand that I will not become a member of Mission94 by submitting this membership application and paying the associated fees and that Mission94’s acceptance of my membership application and the associated fees does not guarantee that I will become a member. I must satisfy the conditions listed above before I become a member of Mission94.
